Fees for Service
- Initial intake visit up to 60 minutes $500.00
- Routine follow up or med management visit up to 30 minutes $300.00
- Monthly refill only visits for controlled* medications $250.00 by the third visit without dosage or brand medication changes
- Diagnostic letter, doctor’s note, or draft less than 15 minutes $100.00
- Documentation that requires 16 to 30 minutes $300.00
- Laboratory orders and review $100.00
- Prior authorizations for medication approval, if needed, $100.00
- Urgent or emergency visits within 72 hours $600.00
